This page includes downloadable applications for High School Baseball, Youth Baseball and Softball programs.

Step 1: Decide which camp you will attend.

Step 2: If the camp has multiple sessions, be sure to pick the session number, day and time.

Step 3: Click to download the appropriate application form (High School Baseball, Youth Baseball, Softball), below, to download the registration form.

Step 4: Fill-in all the appropriate sections of this registration form.

Step 5: Then select one of the following options for payment and registration:

Attach a check to the completed registration form and mail to the Haymarket Sportsplex.
Email a scanned copy of the completed registration form and call us with your credit card information.
